Job Summary:

Northwestern Memorial Healthcare is seeking a highly motivated and compassionate Organ Transplant Nurse Specialist to join our team. As an integral member of the interdisciplinary team, this nurse will provide expert care to patients undergoing organ transplantation, focusing on coordination, education, and support.

About the Position:

The Organ Transplant Nurse Specialist will work closely with the healthcare team to develop and implement individualized care plans, ensuring patients receive comprehensive and timely care. Key responsibilities include coordinating patient care, providing education and support, and collaborating with the interdisciplinary team to optimize patient outcomes.

Responsibilities:
  • Coordinate patient care, including scheduling appointments, tests, and procedures
  • Develop and implement individualized care plans, incorporating patient preferences and goals
  • Collaborate with the healthcare team to provide education and support on transplant-related topics
  • Monitor patient progress, identifying areas for improvement and making recommendations for change
Qualifications:
  • Bachelor of Science in Nursing (BSN)
  • Licensed Registered Nurse (RN) in the state of Illinois
  • CPR-Basic Life Support (BLS)
  • Minimum 2-3 years of recent experience in transplant nursing or a related field
Salary Range:

$85,000 - $110,000 per year
