About the Opportunity:

This is an exciting opportunity to join Northwestern Medicine as a Transplant Nurse Coordinator, where you will play a critical role in ensuring the highest level of patient care and satisfaction.

Key Responsibilities:

  • Coordinate patient care throughout all phases of transplantation
  • Develop and implement individualized care plans
  • Collaborate with the interdisciplinary team to ensure seamless delivery of care
  • Provide education and support to patients, families, and caregivers
  • Contribute to quality and process improvement initiatives

Requirements:

  • Bachelor of Science in Nursing (BSN)
  • Licensed Registered Nurse (RN) in the state of Illinois
  • CPR-Basic Life Support (BLS)
  • Minimum 2-3 years of recent & related clinical experience

Salary Information: $95,000 - $125,000 per annum
