Qualifications:

High School diploma or equivalent is mandatory
A minimum of 6 months' relevant experience is essential
Data entry proficiency of 25-40 keystrokes per minute is required, with 50 WPM preferred
Basic computer literacy and proficiency in Microsoft Excel and Access
Intermediate skills in Microsoft Word and Outlook

Responsibilities:

Respond to inbound communications including calls, emails, and faxes in a professional and timely manner
Initiate placement requests and respond to inquiries regarding treatment options through outbound communications
Conduct outbound calls to healthcare facilities to finalize placement requests

Collaborate with Field Operations to establish and strengthen relationships with healthcare providers.

Adhere to established performance metrics and quality assurance standards
Utilize available tools and resources to effectively coordinate and complete patient placements
Maintain and enter data from forms received from the field into the appropriate database
Under supervision, enhance processes to promote effective growth in placements and treatment options
Foster positive relationships with healthcare facilities, discharge planners, case managers, and colleagues to ensure service excellence
Meet departmental metrics and goals
Be available to work during expanded hours to accommodate patients and colleagues across different time zones
Comply with all regulations regarding patient information to ensure confidentiality
Demonstrate a willingness to learn about kidney disease and related subjects
