Position Overview
Presbyterian Homes & Services is in search of a dedicated Registered Nurse to become a vital part of our RAI/MDS team. This role primarily involves day shift hours, with flexibility to accommodate business needs, and operates within a Monday to Friday schedule, ensuring no weekend commitments. There is no on-call duty, and responsibilities are strictly confined to the MDS role. You will provide support to designated facilities while having the option to work from a PHS location that is convenient for you, subject to available office space.
Key Responsibilities
The MDS Nurse will be accountable for the completion, coordination, implementation, and monitoring of the comprehensive RAI-Resident Assessment Instrument (MDS) along with related assessments, and for formulating resident care plans that align with regulatory standards and established clinical best practices.
- Collaborating closely with the RAI Director to finalize MDS assessments for assigned residents.
- Accurately completing the MDS, Care Area Assessments (CAA's), and comprehensive care plans in accordance with RAI guidelines and within specified time frames.
- Engaging with interdisciplinary team members to gather essential clinical information.
- Utilizing chart audits, resident and family interviews, and communication with direct care staff to ensure thorough documentation.
- Participating in daily interdisciplinary team phone calls.
- Ensuring that care plans accurately reflect the services provided.
Qualifications
- Current licensure as a Registered Nurse with the State Board of Nursing in the relevant states.
- Previous nursing experience in long-term care, transitional, or acute care settings; prior MDS experience is preferred.
- Demonstrated alignment with the mission and operational philosophies of PHS.
- Proven ability to read, write, speak, and understand the English language for effective communication with all stakeholders.
