We are looking for a highly skilled Patient Support Specialist to join our team in Portland, ME. As a Patient Support Specialist, you will provide critical support to veterans and their families through case management and counseling services.
Key Responsibilities- Conduct comprehensive assessments to identify patient needs and develop personalized care plans.
- Provide education and support to patients and their families on health-related topics.
- Collaborate with healthcare providers to ensure seamless care coordination.
- Masters degree in social work from a CSWE-accredited program.
- Licensure or certification as a social worker at the independent practice level.
- Experience working with diverse populations, including veterans with mental health conditions, substance abuse issues, and other complex needs.
- A competitive salary range of $62,969 - $99,040 per year.
