We are seeking a skilled and compassionate Care Coordinator for Gastroenterology Clinic to join our team at Northern Light Health.
Job Summary
The Care Coordinator will be responsible for delivering high-quality patient care, promoting health and safety, and serving others in a fast-paced environment. As a member of our interdisciplinary team, you will work closely with healthcare professionals to ensure seamless coordination of patient care.
Responsibilities
- Develop and implement individualized care plans that meet the unique needs of patients
- Collaborate with healthcare teams to ensure effective communication and handoffs
- Provide education and support to patients and families to promote self-care and empowerment
- Maintain accurate and up-to-date records of patient interactions and progress
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Perform other duties as assigned by management
Requirements
To be successful in this role, you must possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment. You should also have a strong commitment to providing high-quality patient care and a passion for continuous learning and professional growth.
Education and Certification
A minimum of a High School Diploma or equivalent is required, with a focus on nursing or a related field. Current registration as a Registered Nurse (RN) is also required.
Work Environment
This is a full-time position working 36 hours per week, with alternating shift schedules including day, evening, nights, and weekends. The successful candidate will be required to lift, move, and load objects weighing up to 50 pounds.
Salary
The estimated salary for this position is $63,500 - $83,400 per year, based on experience and qualifications.
