Hospice Care Nurse

Care Partners is seeking a dedicated Registered Nurse to join our team as an Admission Nurse in the Portland metropolitan area. This role involves providing skilled nursing assessment and planning, as well as coordination and care management across various settings, including private homes, adult foster homes, residential/assisted living care, and nursing facilities.
Key Responsibilities:
- Conduct thorough patient assessments and develop individualized care plans in collaboration with the interdisciplinary team.
- Provide high-quality nursing care, addressing the physical, emotional, and spiritual needs of patients and their families.
- Counsel patients and families on symptom management, medication, and other direct nursing care measures.
- Collaborate with other healthcare providers to ensure seamless care coordination and optimal patient outcomes.
- Document patient records accurately and maintain up-to-date information in accordance with policies and procedures.
Requirements:
- Current Oregon licensure as a Registered Nurse.
- At least one year of professional nursing experience in hospice, palliative care, home health, or medical-surgical care.
- Valid driver's license and reliable transportation for patient visits.
- Strong critical thinking, patient assessment, and problem-solving skills.
- Ability to work independently and as part of an interdisciplinary team.
