Hospice Case Manager RN Job Description

Overview:

We are seeking a compassionate and skilled Hospice Case Manager RN to join our team at AgeRight Care at Home. As a Hospice Case Manager RN, you will play a vital role in delivering person-centered care to patients in their time of need.

Responsibilities:

  • Conduct patient visits and assessments to understand their unique needs and preferences.
  • Develop and implement personalized care plans to manage pain and symptoms, ensuring the highest quality of life for our patients.
  • Provide education and support to patients, families, and facility staff to promote understanding and empowerment.

Requirements:

  • Must be a licensed RN in good standing with the state of Oregon.
  • Possess strong computer skills and the ability to adapt to different social, racial, cultural, and religious modes.

Preferred Qualifications:

  • Experience working in Hospice care.
  • Able to make accurate, at times, quick judgements.
