Job Summary

Care Partners is seeking a dedicated Hospice Aide to provide compassionate care to patients in their homes. As a member of our interdisciplinary team, you will work closely with nurses and other healthcare professionals to deliver holistic services and support to patients and their families.

Key Responsibilities:

  • Provide personal care to patients in accordance with individualized plans of care and visit frequencies assigned.
  • Assist patients with bathing, oral care, skin care, and ambulation, as well as turning and positioning patients for comfort.
  • Report changes in patient condition and needs to the care team.
  • Maximize patient comfort, safety, and health by adhering to hospice philosophy and organizational policies.
  • Maintain accurate and up-to-date patient records in accordance with policies and procedures.

Requirements:

  • A minimum of one year of prior CNA, Hospice Aide, or equivalent work experience required.
  • A current certificate of practice as a Certified Nursing Assistant (CNA) in the state of Oregon is required.
  • Basic Life Support CPR card for Health Care Provider and First Aid certification are also required.
  • A valid driver's license and acceptable driving record are necessary for travel to and from patient homes and/or care residences.
  • Demonstrated skill in communication, patient documentation, and service coordination is essential.
  • Bi-lingual, Spanish speaking ability is a plus.
