Job Title: Hospice Aide - Full Time

Care Partners is seeking a dedicated and compassionate Hospice Aide to join our team in the Portland metropolitan area. As a Hospice Aide, you will provide personalized care to patients in their homes, focusing on their physical, emotional, and spiritual well-being.

Responsibilities:
  • Provide patients with personal care, including bathing, grooming, and ambulation, in accordance with their individualized plan of care.
  • Assist patients with daily living activities, such as eating, dressing, and using the bathroom.
  • Report changes in patient condition and needs to the care team.
  • Maximize patient comfort, safety, and health by following hospice philosophy and organizational policies.
  • Maintain accurate and up-to-date patient records.
Requirements:
  • A minimum of one year of prior CNA, Hospice Aide, or equivalent work experience.
  • A current certificate of practice as a Certified Nursing Assistant (CNA) in the state of Oregon.
  • Basic Life Support CPR card for Health Care Provider and First Aid certification.
  • A valid driver's license and acceptable driving record.
  • Excellent communication and documentation skills.
  • Ability to work independently and as part of a team.
