Social Services Specialist

At Laconia Rehabilitation Center, we are seeking a compassionate and skilled Social Services Specialist to join our interdisciplinary team. As a Social Services Specialist, you will work closely with patients, residents, and their families to identify strengths, social, emotional, and mental health needs. Your goal will be to provide patients with the highest practical level of physical, mental, and psychosocial well-being and quality of life.
Key Responsibilities:- Collaborate with the interdisciplinary team to promote and protect patient rights and well-being.
- Develop and implement written, interdisciplinary plans of care to address social service needs.
- Hold meetings with patients, families, and significant others to provide support and information.
- Perform discharge planning activities, including family education, nursing center documentation, and referrals to community-based agencies.
- Establish relationships with community-based agencies and services to benefit our patient population.
- $21.00 per hour
