Position Overview

This is a full-time role focused on providing essential support and care to patients and their families in a hospice setting.

Key Responsibilities

The Hospice Medical Social Worker plays a vital role in assessing and addressing the emotional and social needs of patients and their loved ones. Key responsibilities include:

  • Evaluating the need for grief counseling and bereavement support for patients and families.
  • Informing families about available resources to meet their needs.
  • Conducting psycho-social assessments and documenting visit outcomes in accordance with established protocols.
  • Contributing to the development and continuous assessment of the psycho-social aspects of care plans.
  • Providing short-term therapeutic counseling and casework services during the admission period and up to one year post-death.
  • Assisting team members in understanding the social, emotional, and ethical factors affecting patient care.
  • Facilitating discharge planning when hospice care is no longer suitable.
  • Guiding families in adjusting living arrangements as necessary.
  • Connecting patients and families with community resources and financial assistance.
  • Maintaining a productivity standard of three visits per day.
  • Participating in interdisciplinary meetings and case conferences as required.
  • Engaging in quality improvement initiatives and educational activities for staff and community members.
  • Being available for weekend, holiday, and on-call rotations.

Additional Responsibilities

The Hospice Medical Social Worker also mentors new staff and students, providing guidance and support as they acclimate to their roles. Reporting to the Hospice Care Services Manager, this position is crucial in delivering compassionate care to patients and families in home care and hospice settings.

Qualifications

To be considered for this role, candidates must possess:
  • A Master of Social Work (MSW) degree from an accredited institution.
  • A minimum of one year of experience in a healthcare environment.
