Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at Laconia Rehabilitation Center in Laconia, NH. As a Social Worker, you will play a vital role in enriching the lives of our patients/residents by identifying their strengths, social, emotional, and mental health needs, and obtaining services to meet those needs.

Key Responsibilities
  • Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
  • Develop and implement written, interdisciplinary plans of care for each patient/resident to address social service needs.
  • Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
  • Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers.
  • Establish relationships and maintain contact with community-based agencies/services that benefit our patient/resident population.

Qualifications

To be successful in this role, you must possess:

  • Bachelor's degree in social work or human services field
