Position Overview: The Clinical Care Coordinator for Inpatient Services plays a pivotal role in the healthcare delivery process, working closely with a multidisciplinary team to assess, plan, implement, and evaluate patient care. This position is essential in addressing the clinical, psychosocial, and familial aspects of each patient's care and disease management strategy.

Key Responsibilities:

  • Facilitate effective communication and manage transitions of care, assisting patients and their families in navigating the complexities of the healthcare system.
  • Identify and secure necessary resources to ensure the safety and well-being of patients and their families throughout the continuum of care.
  • Collaborate with financial payers and insurers to obtain resources vital for patient care.
  • Utilize a Bachelor's degree in Nursing and a minimum of two years of experience in an acute care facility, focusing on clinical specialties or care management.
  • Conduct thorough assessments of patients, considering clinical, psychological, and social factors that may impact their health and care needs.
  • Review admission histories and maintain accurate documentation in the electronic medical record (EMR).
  • Engage with patients, healthcare providers, social workers, and community resources to enhance case management and service delivery.
  • Provide necessary documentation to third-party payers to demonstrate medical necessity.
  • Support patients and families through care planning, safety evaluations, and transitional care arrangements.
  • Conduct utilization reviews to ensure adherence to evidence-based clinical guidelines for appropriate care levels.
  • Identify when medical necessity criteria are not met and communicate this through appropriate documentation.
  • Apply organized and purposeful thinking to make informed clinical judgments and decisions.
  • Document and evaluate financial, clinical, functional, and satisfaction outcomes as patients progress towards their goals.
  • Address the educational needs of clients, clinicians, and the community regarding discharge planning and utilization review.
  • Manage care and utilization review for patients with varying complexities, optimizing time and resources.
  • Provide leadership and guidance to the healthcare team in discharge and care planning processes.
