Early Childhood Services Coordinator

Overview:

The Early Childhood Services Coordinator is responsible for engaging and guiding families in the Help Me Grow program.

Key Responsibilities:

  1. Engage and guide families interested in the Help Me Grow initiative.
  2. Conduct regular home visits to support parents and children in their developmental journey.
  3. Ensure all developmental assessments are completed and refer families for further evaluation if any delays are identified.
  4. Assist in the creation, assessment, and monitoring of the Individualized Family Service Plan (IFSP) to achieve desired family outcomes.
  5. Identify specialized services and resources, providing families with information on advocacy options.
  6. Coordinate and oversee the delivery of services, collaborating with healthcare providers.
  7. Facilitate transitions to additional programs and services as needed.
  8. Ensure all necessary data is entered into the web-based Early Track system.
  9. Maintain accurate records in the Help Me Grow database and complete all required documentation.
  10. Participate in weekly individual and team supervision sessions.
  11. Contribute as a member of the developmental evaluation team to assist in determining eligibility for Part C services when required.
  12. Exhibit the knowledge and skills necessary to address the physical, psychological, social, educational, and safety needs of children aged 0 to 3.
  13. Perform additional duties as assigned.

Qualifications:

Technical Skills:

  1. Experience working with families and children aged 0 to 3 is essential.
  2. Experience with diverse cultural backgrounds is preferred.
  3. Familiarity with community resources available to families is advantageous.
  4. Travel and a valid driver's license are necessary.
  5. Experience collaborating with various organizational levels is preferred.
  6. Background in healthcare is a plus.
  7. Proficiency in MS Office applications (Outlook, Excel, Word) or similar software is required.

Education and Experience:

  1. Education: A Bachelor's degree in Social Work, Early Childhood Education, or a related field is required; a Master's degree is preferred.
  2. Certification: A Service Coordinator Credential must be obtained within one year of employment.
  3. Relevant experience: 3 to 5 years is preferred.
  4. Supervisory experience: None required.

This is a full-time position with remote work options available.
