We are currently seeking a Supportive Care Coordinator to join our team


Job Description:

This role involves working as part of a treatment team to provide direct service to youth ages 0-21. The successful candidate will be responsible for tasks such as transporting youth, providing concrete services, modeling behaviors, and performing clerical duties.


Essential Functions:
  1. Reports to the designated supervisor and works collaboratively with the treatment team.
  2. Provides direct service to youth, including transportation and concrete support.
  3. Assists with activities of daily living and personal care for youth in their care.
  4. Acts as a role model and mentor to youth through assigned activities.
  5. Maintains accurate records and documentation, adhering to Agency and DCFS policies.
  6. Participates in training and professional development opportunities to enhance skills and knowledge.

Requirements:
  • High School Diploma or equivalent; Bachelor's degree preferred.
  • Satisfactory completion of at least one year as a child care worker or related experience.
  • Ability to work effectively with children and young adults, reading and writing sufficiently to follow directions and maintain written records.
  • Computer proficiency and good communication skills.
  • Familiarity with childhood trauma and prior experience working in settings with youth and families exposed to trauma or experiencing traumatic stress (preferred).

Estimated Salary:

$45,000 - $60,000 per annum, depending on experience.
