We are seeking a skilled Pediatric Care Coordinator to join our team at La Rabida Children's Hospital. As a key member of our healthcare team, you will play a vital role in ensuring the optimal health and well-being of our pediatric patients.

Job Overview:

The Pediatric Care Coordinator is responsible for providing comprehensive care coordination services to pediatric patients with complex medical conditions. This includes evaluating patient needs, developing and implementing personalized care plans, and facilitating communication between patients, families, and healthcare providers.

Responsibilities:

  • Evaluating patient needs and developing personalized care plans
  • Coordinating with healthcare providers to ensure seamless care transitions
  • Facilitating communication between patients, families, and healthcare providers
  • Monitoring patient outcomes and adjusting care plans as needed
  • Providing education and support to patients and families on disease management and self-care techniques

Requirements:

  • Graduate of an approved school of professional nursing (BSN required)
  • Three years of pediatric nursing experience
  • Certification in Pediatric Nursing or Case Management preferred
  • Excellent written and verbal communication skills
  • Demonstrated knowledge and expertise in pediatrics
