Public Health Specialist

We are seeking a highly motivated and experienced Lead Home Visitor to join our team at the New York City Department of Health and Mental Hygiene. As a Lead Home Visitor, you will play a critical role in improving maternal and child health outcomes by providing home visiting services to eligible mothers and families.
Job Summary:The Lead Home Visitor will conduct home visits to deliver prenatal and postpartum health education, provide assessments and instruction on maternal and infant health, and connect families to internal program specialists and community resources. The ideal candidate will have experience in home visiting or community outreach, knowledge of maternal and child health, and strong written and verbal communication skills.
Responsibilities:- Conduct home visits to eligible mothers and deliver prenatal and postpartum health education using a standard curriculum, referral, and data collection process.
- Provide assessments, instruction, and health education on maternal and infant health, including breastfeeding, safe sleep, mental health, and chronic illness.
- Conduct assessments of the home environment for health hazards such as lead and make appropriate referrals for follow-up.
- Collect, monitor, input, review, and track data on case assignments.
- Assess and connect families to internal program specialists and community resources.
- Make post-visit phone calls to ensure follow-up with external referrals has been made.
- Visit community-based and clinical providers to identify and offer visits to eligible families.
- Participate in marketing programs through community events, conduct neighborhood health assessments and/or surveys, and assist senior management in other assignments.
- A baccalaureate degree from an accredited college, including or supplemented by twelve semester credits in health education, or in health, social, or biological sciences.
- Or a baccalaureate degree from an accredited college, and six months of full-time satisfactory experience in a health promotion or disease intervention/prevention program, performing one or more of the following: interviewing, conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data.
- Or a four-year high school diploma or its educational equivalent, and four years of full-time satisfactory experience as described in the above experience requirement.
- Experience in home visiting or community outreach.
- Knowledge of maternal and child health, and health education.
- NYS driver's license.
- Strong written and verbal communication skills.
- Fluency in English and Spanish or French preferred.
