About the Role:

We are seeking a skilled Case Manager, Transitional Housing to join our team at Breaking Ground. As a key member of our staff, you will provide direct services to individuals experiencing homelessness, including conducting assessments, benefits assistance, and connecting clients to psychiatric services, medical treatment, and substance use treatment facilities.

Key Responsibilities:

  • Provide case management services to a caseload of approximately 15 clients, including conducting necessary assessments and connecting clients to relevant services.
  • Conduct psychosocial evaluations and develop service plans to support clients in achieving housing and recovery goals.
  • Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans.
  • Prepare documentation for housing applications and assist clients in the process of obtaining permanent housing.
  • Maintain record keeping in compliance with agency standards and City and State agencies.
  • Intervene in crisis situations and escort clients to appointments as needed.

Requirements:

  • Bachelor's Degree or equivalent work experience.
  • Two years of experience working with homeless adults and substance abuse issues preferred.
  • Experience with harm reduction and motivational interviewing techniques strongly preferred.
  • Valid NY or NJ driver's license with a good driving record, as defined by Breaking Ground is preferred.
  • Excellent computer skills, including proficiency in Microsoft Word and Excel and the ability to learn new programs.
  • Excellent writing and communication skills.
  • Bilingual Spanish/English is preferred.

What We Offer:

A 35-hour work week and a competitive hourly rate of $24.73.
