Job Summary:

The Patient Navigator Women's Health Specialist will assist patients and their families in navigating the healthcare system to meet their health and wellness needs. This role requires a high level of empathy and understanding of the patient's perspective, as well as excellent communication and interpersonal skills. The ideal candidate will be able to provide personalized care and support to patients, while also working collaboratively with the healthcare team to ensure seamless care coordination.

Key Responsibilities:

* Assist patients and their families in navigating the healthcare system
* Provide personalized care and support to patients
* Collaborate with the healthcare team to ensure seamless care coordination
* Develop and maintain relationships with patients and their families
* Participate in health screenings and provide patient education

Requirements:

* Bachelor's Degree in Social Work or related field
* National Provider BLS - American Heart Association upon hire
* 1 year of experience in a healthcare setting
