Job Summary

We are seeking a skilled Navigator to join our team at Community Health Centers of the Central Coast. The Navigator will play a critical role in linking patients with Behavioral Health services, supporting clinics with increased access to quality and timely healthcare.

Key Responsibilities
  • Conduct care coordination activities, behavioral health maintenance, and link patients with resources in the community.
  • Use a remote care monitoring platform to facilitate patient engagement and support.
  • Assist patients with completing forms, verifying insurance and demographic information, and ensuring chart accuracy.
  • Identify gaps in care and implement quality metrics using disease management guidelines.
  • Collaborate with the care team to maintain a continuous relationship with patients and provide wellness education.
  • Perform primary, secondary, and tertiary prevention strategies, including screening and referral to BH providers and resources.
  • Provide quality customer service, answering telephones, and scheduling appointments.
  • Participate in service coordination activities to improve service continuity and effectiveness.
Requirements
  • High school diploma or GED required; Associate's degree or related field preferred.
  • Experience in front and/or back office medical and/or behavioral health required.
  • Strong communication skills, medical terminology, and problem management strategies.
  • Ability to work in a fast-paced environment and adapt to changing priorities.
  • Knowledge of domestic violence, child and dependent abuse protocols, and cultural sensitivity.
