Reunification Worker Job Description

At Professional Services Group & Community Impact Programs, we are seeking a dedicated and compassionate Reunification Worker to join our team. As a Reunification Worker, you will play a vital role in supporting families in our community by creating care plans to address their unique needs and promote reunification.

Key Responsibilities:
  • Conduct daily face-to-face contact with families to ensure safety and cooperation with the safety plan.
  • Promote parenting skills with families and provide support to help them achieve their goals.
  • Monitor client school attendance and analyze client situations to determine the necessary services required to meet their treatment needs.
  • Formulate, implement, evaluate, and review client treatment plans to ensure the best possible outcomes.
  • Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
  • Act as a resource to evaluate and determine necessary responses in de-escalating crisis situations with youth and families.
  • Monitor compliance with conditions of the aftercare plan by conducting daily curfew checks, home visits, random urine analysis, and assisting in employment search.
  • Report client behavior to courts and Kenosha County Department of Human Services and make appropriate recommendations.
  • Safely transport clients as needed.
Requirements:
  • Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance.
  • Must have at minimum a Bachelor's Degree in Social Work, Psychology, Criminal Justice, or related.
