Care Coordinator

EverHeart Hospice is seeking an experienced Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and supporting our Medical Director.
Job Summary:
- Oversee the implementation of patient care plans, collaborating with physicians and other healthcare professionals as needed.
- Prescribe medications and update patient records accordingly.
- Participate in palliative care consults, assessing patients' needs and coordinating care to enhance quality of life.
- Provide clinical expertise to the interdisciplinary team, ensuring continuity and comprehensiveness of care.
Requirements:
- Masters Degree in Nursing (MSN) from an accredited School of Nursing.
- Current Nurse Practitioner license in the State of Ohio.
- Specialization in Family Practice, Oncology, or Hospice preferred.
- Minimum 5 years of nursing experience, including clinical care in oncology, hospice, and geriatrics.
Estimated Salary: $120,000 - $150,000 per year, depending on experience.
