About the Job

This LPN position offers a unique opportunity to work with patients and their families in a home-based setting. The successful candidate will be responsible for providing high-quality patient care, coordinating with healthcare professionals, and ensuring seamless transitions in care.

Key Responsibilities:

  1. Deliver compassionate and individualized care to patients, focusing on their physical, emotional, and psychological needs.
  2. Develop and implement personalized care plans, working closely with multidisciplinary teams to achieve optimal outcomes.
  3. Manage patient symptoms, provide education, and promote self-care techniques to enhance quality of life.
  4. Maintain accurate records, communicate effectively with colleagues, and contribute to a positive team environment.

Qualifications:

  • Diploma or degree in Nursing
  • Licensure to practice as an LPN in Ohio
  • At least two years of experience in a related field (Medical/Surgical, Oncology, or community health nursing)
  • Hospice experience is desirable but not essential

Salary Range: $60,000 - $70,000 per year, commensurate with experience
