Job Description

The Primary Nurse plays a vital role in delivering direct patient care as a Home Care RN Specialist at Hospice of Darke County.

Key Responsibilities:
  • Provide skilled symptom management, emotional support, and timely response to patient and family needs.
  • Coordinate IDG planning for assigned patients, analyzing their needs and developing tailored plans of care.
  • Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers.
  • Facilitate continuity of care by coordinating admission of home-care patients to in-patient settings when necessary.
Requirements:
  • Graduation from an approved School of Nursing.
  • Current RN licensure in the State of Ohio.
  • Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing.
  • Hospice experience preferred.
Working Conditions:
  • Ability to work independently without supervision.
  • Travel to other sites as needed, including potential travel in inclement weather.
  • Ability to lift 25 lbs of supplies or equipment.
  • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
