We are seeking a highly motivated and experienced Nurse Practitioner to join our team as a Hospice Care RN. As a member of our interdisciplinary team, you will provide direct patient care in the Hospice Care Center, working collaboratively with other healthcare professionals to ensure the highest level of comfort and dignity for our patients and their families.

Job Summary: This is an excellent opportunity for an experienced nurse practitioner to make a meaningful difference in the lives of our patients and their loved ones.

Key Responsibilities:

  • Provide comprehensive patient care, addressing physical, emotional, and spiritual needs.
  • Develop, implement, and revise the IDG Plan of Care in collaboration with the IDG team.
  • Supervise and evaluate the care provided by volunteers, hospice aides, and primary caregivers.
  • Facilitate continuity of care by coordinating transfer/admission of home-care patients to in-patient setting and vice versa.
  • Explain hospice services and benefits to patients and families, including Medicare, Medicaid, and Private Insurances.

Requirements:

  • Graduation from an approved School of Nursing.
  • Current RN licensure in the State of Ohio.
  • Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.
  • Hospice experience preferred.

Salary: $75,000 - $95,000 per year, depending on experience.
