Housing Outreach Specialist

We are seeking a highly skilled Housing Outreach Specialist to join our team at Elevance Health. As a key member of our housing team, you will be responsible for developing and implementing our health plan's housing strategy and programs.

Key Responsibilities:

  • Liaise between housing agencies and our health plan to develop and access affordable housing services for our members.
  • Partner with our Healthy Blue lead trainers to develop and implement training specific to affordable housing services in assigned states.
  • Work under the Housing First model, honoring member choice.
  • Responsible for working with housing agencies and other housing programs to help develop and access affordable housing services for members receiving long-term services and supports.
  • Responsible for educating and supporting care coordinators and support coordinators in identifying housing options for members.

Requirements:

  • Requires a minimum of 3 years' experience in developing housing and/or housing policy at a local, state, or federal level for special populations such as the elderly, people with disabilities, and/or people with mental health challenges.

Preferred Skills:

  • A bachelor's degree in social services or a related field is highly preferred.
  • Experience in behavioral health is preferred.
  • Substance abuse/peer support is preferred.

Elevance Health operates in a hybrid workforce strategy. Associates are required to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process. Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location.
