Job Summary
This full-time Hospice Nurse Case Manager position offers a rewarding opportunity to work with our interdisciplinary team to provide comprehensive end-of-life care to patients in their homes or residential facilities. As a key member of our team, you will assess patient needs, develop individualized care plans, and coordinate services with other healthcare providers to ensure seamless care transitions.

Responsibilities
- Assess patient physical, psychological, social, and spiritual factors that impact their care needs
- Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team
- Coordinate services with other healthcare providers to ensure optimal care delivery
- Provide regular visits to reassess patient needs and update care plans as necessary
- Perform clinical pronouncement of death and provide support to the family

Requirements
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Current CPR and First Aid certification
- Knowledgeable of quality assessment and assurance procedures
