Job Title: Integrated Behavioral Health Consultant

We are seeking a highly skilled and compassionate Integrated Behavioral Health Consultant to join our team at Waterfall Community Health Center. As a key member of our Primary Care team, you will provide trauma-informed and evidence-based care to patients with social, behavioral, and emotional factors that influence physical health.

Job Summary:
  • Provide short-term counseling and consultation to meet the needs of patients with social, behavioral, and emotional factors that influence physical health.
  • Conduct screenings, provide consultation and psychoeducation, make brief assessments and discuss interventions, provide referrals, and contribute to care coordination with individuals, families, and groups across the lifespan.
  • Work collaboratively with the Primary Care team to provide comprehensive behavioral services and support program goals to reduce cultural and socio-economic barriers between clients and institutions.
Responsibilities:
  • Adhere to the Waterfall Community Health Center's mission and values while performing assigned duties.
  • Provide trauma-informed and evidence-based counseling services within a Primary Care setting.
  • Keep accurate and timely records, including comprehensive documentation of all patient interactions.
  • Communicate effectively with patients, coworkers, and supervisors.
  • Foster a supportive, trauma-informed, and growth-centered approach with patients, staff, and the community.
Requirements:
  • Masters Degree in Counseling, Social Work, Psychology, or a related field.
  • Ability to become registered with the Mental Health and Addiction Certification Board of Oregon (MHACBO) as a Qualified Mental Health Professional or hold an Unrestricted Oregon License and hold credentials as an LCSW, CSWA, LPC, LPC Registered Associate, LMFT, PsyD, or Ph.D.
  • Non-violent Crisis Intervention Comprehensive training class completed within 6 months of hire and recertify prior to the expiration date.
