We are seeking passionate individuals to join our Outreach Services team in San Diego as Client Support Specialists.

ABOUT OUR ORGANIZATION
Since 1984, our organization has implemented innovative and effective strategies to assist individuals facing homelessness. We provide a wide range of services across California, connecting clients to essential resources including homelessness prevention, street outreach, employment preparation, individualized case management, and both interim and permanent supportive housing.

ROLE SUMMARY
The primary responsibility of the Client Support Specialist is to help clients break the cycle of homelessness through a comprehensive and tailored approach. This role supports clients at various stages of homelessness, including street outreach, interim housing, transitional housing, and permanent housing. The Specialist aids clients in accessing necessary services, including healthcare, social services, and support systems, while navigating through a coordinated entry system. Individualized support will be provided to help each client create a plan that addresses their strengths, barriers, health needs, income enhancement, and housing stability. Key responsibilities include:

  • Conducting intake assessments and collaborating with clients to develop Individualized Service Plans (ISPs) that focus on housing objectives, income maintenance, and other personal goals, including medical, mental health, and vocational needs.
  • Providing referrals and support to resources that assist clients in achieving their ISP goals.
  • Completing specific program assessments.
  • Evaluating client strengths and challenges to meet both short-term and long-term objectives, including conducting follow-up reviews.
  • Maintaining accurate documentation as per program and organizational policies, ensuring timely case file updates.

QUALIFICATIONS

PREFERRED EXPERIENCE

  • Two years of experience in healthcare, case management, or working with vulnerable populations.

MINIMUM REQUIREMENTS

  • All levels of education and experience are encouraged to apply.

MANDATORY CRITERIA
All candidates must:

  • Provide employment eligibility verification.
  • Obtain CPR/First Aid certification.
  • Successfully complete a tuberculosis test, background screening, and drug test as a condition of employment.

DRIVING REQUIREMENTS
Driving is a crucial function of this role. Candidates must possess:

  • Reliable transportation.
  • A valid driver's license.
  • Proof of insurance and ownership for personal vehicles used for work.
  • Eligibility for insurance coverage.
