Serenity Hospice is seeking a dedicated and compassionate Admissions RN to join their team in providing exceptional care to patients and their families. As an Admissions RN, you will play a vital role in educating and informing patients and families about our services, and initiating the admission process to optimize patient and family-directed comfort and quality of life.
Responsibilities:
- Provide nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.
- Initiate the admission process by educating and informing patients/families of our services.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Ensure compliance with regulatory requirements and quality assurance procedures.
