Key Responsibilities:
This position, under supervision, involves assisting clients in developing individualized program plans, managing client cases, and adhering to program protocols. The role emphasizes the application of evidence-based methodologies and supporting the training of fellow case managers.
Main Duties Include:
- Oversee client intakes, regular check-ins, and essential housekeeping tasks.
- Develop tailored programs for clients to reach their personal goals and enhance life skills.
- Assist clients with budgeting, housing, and employment initiatives.
- Monitor safety within the facility and client activities.
- Provide orientation and guidance to clients for successful reintegration into the community.
- Document client progress for effective program planning and evaluation.
- Verify client employment status and oversee pass plans.
- Prepare reports upon client discharge and participate in team meetings.
- Maintain confidentiality of sensitive client information.
- Ensure client adherence to counseling and program regulations.
- Communicate effectively with team members and comply with organizational policies.
- Follow safety protocols and be available for overtime as required.
- Perform additional duties as assigned.
Qualifications:
- Bachelor's Degree in social sciences or a related discipline.
- At least 1 year of experience in human services or a comparable field.
- Familiarity with community correctional systems and workforce development.
- Strong written communication and mathematical abilities.
- Capacity to respond swiftly in emergency situations.
- Proficiency in computer skills and bilingual capabilities are advantageous.
