Overview of the Position

The primary objective of this role is to deliver consistent daily assistance and care to each resident assigned to you, in alignment with their individual assessments and care plans, as well as directives from supervisory staff.

Authority Delegation

As a Nursing Care Assistant, you are entrusted with the necessary administrative authority, responsibility, and accountability to fulfill your designated tasks.

Key Responsibilities

Administrative Duties

- Utilize identification methods such as wristbands or photo cards to confirm resident identities prior to administering care or services.

- Document all relevant information on flow sheets, notes, and charts in a clear and informative manner.

- Adhere to authorized abbreviations established by the facility when recording information.

- Communicate any changes in a resident's condition to the Nurse Supervisor or Charge Nurse promptly.

- Report any incidents or accidents observed during your shift immediately.

Admission, Transfer, and Discharge Duties

- Prepare the resident's room for their arrival, ensuring all necessary items are in place.

- Welcome residents and guide them to their designated rooms.

- Introduce residents to their roommates and other staff members as appropriate.

- Assist with the organization of the resident's personal belongings as instructed.

- Aid residents in packing their belongings during transfers or discharges.

- Facilitate the transfer of residents to new rooms or designated areas.

- Assist with loading and unloading residents from vehicles as necessary.

Care Functions

- Carry out all assigned tasks in accordance with established policies and procedures, following supervisory instructions.

- Collaborate with other departments to ensure comprehensive nursing services are provided to meet resident needs.

- Foster a warm and inviting atmosphere, promoting a calm environment throughout the unit.

- Regularly meet with nursing personnel to identify and address any service improvement areas.

- Document and report any resident complaints or grievances to the appropriate personnel.

- Notify the facility of any lateness or absences from work as required.

- Report any exposure to hazardous materials or infectious substances to your supervisor.

Personal Care Responsibilities

- Participate in nursing reports at the beginning of your shift.

- Assist residents with daily hygiene tasks, including oral care and bathing.

- Support residents with dressing and grooming as needed.

- Ensure residents are comfortable and dry, changing linens and gowns as necessary.

- Assist with mobility and transportation to appointments and activities.

- Maintain accurate records of residents' intake and output as instructed.

- Provide compassionate care to residents, including those nearing end-of-life.

- Observe and report any changes in residents' conditions or behaviors.

Food Service Duties

- Prepare residents for meals, ensuring their comfort and hygiene.

- Serve meals and assist residents with eating as needed.

- Monitor and document residents' food and fluid intake, reporting any changes.

- Maintain cleanliness of dining areas and assist with post-meal care.

Safety and Sanitation

- Follow established safety protocols in all tasks performed.

- Maintain a clean and organized work environment, reporting any hazards immediately.

- Participate in training programs related to safety and infection control.

Qualifications

- Must possess a minimum of an 8th-grade education.

- Must be a State Certified Nursing Assistant in accordance with New York laws.

- Ability to communicate effectively in English, both verbally and in writing.

- Must demonstrate patience, tact, and a positive attitude when interacting with residents and staff.

- Must be willing to adapt to new methods and practices in nursing care.

- Commitment to maintaining confidentiality and treating all residents with dignity and respect.
