Position Title: Registered Professional Nurse (RN)

Supervisor: Clinic Manager

Summary:

The Registered Professional Nurse plays a vital role in delivering extensive nursing services, which encompass assessment, education, counseling, and preventive care for individuals and families at Alluvion Health. All activities are conducted in compliance with the state of Montana's licensure requirements and the policies of Alluvion Health.

Key Responsibilities:

  1. Coordinate primary health care with the patient's primary care provider and other health center providers as necessary.
  2. Review and update patients' medical records prior to their appointments.
  3. Screen patients and collect relevant information.
  4. Assist in routine physical examinations by preparing patients and necessary equipment.
  5. Execute medical orders for medications, treatments, and procedures as directed by the Provider.
  6. Assess immunization needs, administer vaccines, and maintain accurate immunization records.
  7. Facilitate referrals to community providers and organizations.
  8. Participate in care team meetings.
  9. Ensure efficient patient flow for timely visits.
  10. Communicate treatment recommendations between specialists and primary care providers.
  11. Encourage patient engagement and follow-up care.
  12. Evaluate care plans in collaboration with the care team.
  13. Document patient interactions and progress in the electronic medical record system.
  14. Track referrals for necessary services outside of Alluvion Health.
  15. Engage in meetings and quality improvement initiatives as required.
  16. Serve on committees as needed.
  17. Participate in peer reviews for quality assurance.
  18. Maintain confidentiality in accordance with HIPAA regulations.
  19. Collaborate with staff and community organizations to meet program objectives.
  20. Perform duties efficiently and effectively.
  21. Complete various special projects as assigned.

Qualifications:

Education:

  • Registered Nurse degree required; Bachelor of Science in Nursing preferred.
  • 3-5 years of experience in a primary health care setting or equivalent.

Certifications:

  • State of Montana Nursing License required.
  • Valid driver's license issued by the State of Montana is necessary.

Language Proficiency:

  • Fluency in English, with the ability to read and interpret complex documents and respond to sensitive inquiries.

Physical Requirements:

The physical demands of this role require the employee to:

  • Regularly sit and use hands for patient care and computer tasks.
  • Occasionally lift and move items up to 40 pounds.
  • Have clear vision and hearing for effective communication.
  • Be mobile to navigate various clinic settings.

Working Conditions:

  • Primarily indoors in a climate-controlled environment.
  • Exposure to blood and body fluids as part of routine duties.

Work Hours:

  • Full-time, typically a 40-hour workweek with some evening and weekend hours as necessary.
