Job Title: Medical Care Coordinator

We are seeking a highly skilled and compassionate Medical Care Coordinator to join our team at Ezra Medical Center.

Job Description:

The Medical Care Coordinator will be responsible for collaborating with physicians and assisting in the exam rooms, coordinating services for patients, and working closely with the director of nursing to ensure quality care.

Duties and Responsibilities:

  • Perform selected nursing duties and assist physicians as part of the clinical care team.
  • Update patient charts prior to appointment with recent test results such as blood tests or other diagnostic tests.
  • Confer with physician regarding any incomplete tests or consults prior to patient visit.
  • Prepare patients for examination and treatment by escorting them from the waiting area to the exam room.
  • Obtain and properly document patient care services by charting chief complaint, history, current medications, allergies, vital signs, height, and weight.
  • Maintain nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies, and ensuring that all equipment is properly maintained.
  • Give injections, draw blood, perform EKGs, assist with GYN and Dermatology exams, and assist with collection of specimens and processes them in Ezra Medical's Laboratory as needed.
  • Maintain sample medication records, administer or refill medication upon provider's order.
  • Triage and respond to patient communications within practice policy.
  • Assist with scheduling tests and treatments, pre-certify patients for procedures, and process referrals to other providers as needed.
  • Process prior authorizations for medications and assist providers with Prescription Monitoring Program for patients on controlled substances.
  • Educate patients on provider orders, how to take prescribed medications, and/or use prescribed equipment.
  • Call patients with test results as instructed by provider.
  • Assist with recalls to patients reminding them of the need to schedule well visits or follow-up visits as needed.
  • Establish a compassionate environment by providing emotional and psychological support to patients and families.
  • Perform other related work as required.
  • The Nurse must comply fully with Ezra Medical Center's compliance program, including its standards of conduct and policies and procedures.

Salary: $65,000 - $85,000 per year.
