As a Hospice Care Coordinator, you will be responsible for conducting thorough assessments to admit patients to our hospice program, ensuring that each individual receives the highest level of care.

Job Description:
  • Conduct comprehensive assessments of potential hospice patients to identify their unique needs and develop personalized plans of care.
  • Admit eligible patients into the hospice program, utilizing expert knowledge and skills to ensure seamless patient care.
  • Collaborate with interdisciplinary teams to provide coordinated care and support to patients and families.
Requirements:
  • Minimum 2 years experience in med-surg, community health, oncology or hospice.
  • Graduate of an accredited School of Nursing.
  • NYS Registered Nurse (RN) license required.
Preferred Qualifications:
  • Bachelor of Science in Nursing (BSN).
  • Previous hospice experience preferred.
  • Previous intake or admissions experience desired.

The estimated salary for this position is $85,000 per year, based on location and job requirements.
