Job Title: Care Coordinator

Join the San Francisco Campus for Jewish Living team as a Care Coordinator, where you will play a vital role in delivering exceptional care to our residents. As a key member of our Health and Wellness team, you will be responsible for coordinating care services, supporting staff training, and ensuring the highest quality delivery of care.

Job Summary:

The Care Coordinator will work closely with the Health and Wellness Director and Memory Care Director to support the care team in both assisted living and memory care settings. This role requires a strong focus on customer service, problem-solving, and organizational skills, with the ability to work under time pressures and manage multiple demands simultaneously.

Key Responsibilities:
  • Lead by example, creating a supportive team approach that consistently uses best practices for care delivery.
  • Assist staff in following care-related policies and procedures.
  • Ensure Resident Service Partners assignments are completed and report to management as issues arise.
  • Follow communication and documentation protocols to ensure excellent service to residents.
  • Assist with staff scheduling and coordinate onboarding and training for non-employee homecare agency staff.
  • Provide in-service training and orientation for all resident service partners and staff as needed.
  • Monitor residents for changes in condition and report to management to ensure appropriate care services are delivered.
  • Communicate with residents, family, and staff to meet resident needs and escalate difficult situations to management for resolution.
  • Support the Unit Coordinator and Resident Services Manager as needed.
  • Foster teamwork with co-workers by showing initiative in overcoming obstacles and redirecting challenging resident behaviors.
  • Review daily care logs to ensure all ADLs assignments are met by the care team and report any issues to management.
  • Collaborate with Life Enrichment staff and support/assist the RSP team in providing engaging activities.
  • Document care provided to residents for ADLs, change of condition, and any other event/situation that warrants documentation.
  • Follow safety guidelines in the community, including proper lifting techniques and universal precautions when aiding residents.
  • Transfer or escort residents as needed to onsite or offsite appointments or programs.
  • Respond to resident emergencies consistent with protocol and training.
Requirements:
  • High school diploma or equivalent.
  • Prefer one year of experience providing residential care to the elderly or similar experience with specialized dementia care programs.
  • Must possess above-average human relations, customer service, problem-solving, and organizational skills.
  • Must be able to work under time pressures and manage multiple demands simultaneously.
  • Extreme attention to detail is essential.
  • Excellent telephone and communication skills are necessary.
  • Must be a team player willing to share information and work cooperatively with other members of the management team and caregiving staff.
  • Demonstrated high level of interpersonal skills to handle sensitive and confidential situations.
  • Demonstrated strong computer skills including MS Office, Excel, Word, and electronic health records.
  • Must have the ability to relate professionally and positively and work cooperatively with residents, families, and other employees at all levels.
  • Ability to read, write, communicate, and comprehend verbal and written instructions in English.
  • Must be capable of maintaining regular attendance.
