Key Responsibilities- Coordinate Patient Care: Work with patients, families, care teams, payers, and external entities to promote quality care and ensure a safe transition from our facility to the next provider or care setting.
- Develop and Implement Care Plans: Collaborate with patients, families, and care teams to develop and implement individualized care plans that meet the patient's needs and goals.
- Communicate Effectively: Communicate clearly and concisely with patients, families, care teams, and external entities to ensure effective care coordination.
- Monitor and Evaluate Care: Monitor and evaluate the patient's care and services to ensure quality and safety standards are met.
- Collaborate with Care Teams: Work collaboratively with care teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive and coordinated care.
- Education and Experience: Nursing diploma (associate's or bachelor's degree in nursing) and one year of clinical experience in a post-acute care setting.
- Licensure and Certification: Currently licensed/registered in applicable State and certified as a Case Manager (CCM) or Board Certified in Nursing Case Management (RN-BC).
- Skills and Abilities: Excellent organizational and communication skills, ability to work independently and as part of a team, and knowledge of laws, regulations, and guidelines governing post-acute care facilities.
