Job Summary

The LVN Care Coordinator plays a pivotal role in ensuring patients have access to care, receive appropriate treatment, and coordinates post-operative follow-ups. This position requires strong communication and organizational skills, with a focus on patient-centered care.

Responsibilities
  • Provides patient care in accordance with Department of Nursing standards, preserving personal dignity, autonomy, and cultural values.
  • Complies with universal precaution and infection control policies, maintaining a safe and clean work environment.
  • Employs proper lifting and moving techniques, utilizing assistive devices and equipment safely.
  • Reports unsafe equipment and working conditions, intervening as necessary.
  • Participates in quality improvement activities, offering suggestions to enhance care.
  • Utilizes judgment to establish work priorities, completing assignments with minimal supervision.
  • Observes and reports significant changes in patient condition, responding to emergency situations.
  • Triages patients, performs chart scrubbing, and responds to department phone calls and secure messaging.
  • Coordinates care for patients, including obtaining outside records and communicating with providers.
  • Places and coordinates HOPTEL and Beneficiary Travel Requests, utilizing judgment when communicating with patients.
  • Evaluates and documents patient response to interventions and progress towards goals.
Requirements
  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC).
  • Active, full, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico), of the United States or the District of Columbia.
  • Completion of at least one year of qualifying experience at the GS-5 level or equivalent.
Preferred Experience and Certifications
  • 1-2 years of outpatient services care coordination experience.
Physical Requirements

The physical requirements of this position include lifting, prolonged standing, frequent bending and stooping, walking, reaching, and repetitive motion.

The mental/sensory requirements include recall, reasoning, problem-solving, hearing, and speaking clearly, writing, keyboarding, reading, and logical thinking.

The environment's pace can vary from steady to a sometimes fast pace requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a sometimes noisy environment.
