Care Partners is seeking a highly skilled Behavioral Health Clinician to join our team. As a key member of our interdisciplinary team, you will provide comprehensive mental health services to older adults with complex psychosocial needs.
Key Responsibilities- Crisis Intervention and Management: Provide timely and effective crisis intervention and management to prevent hospitalization and ensure the well-being of our clients.
- Assessment and Treatment: Conduct thorough assessments and provide individual, family, and group therapy to address the unique needs of our clients.
- Case Management: Develop and implement comprehensive service plans to ensure the best possible outcomes for our clients.
- Interdisciplinary Collaboration: Work closely with our interdisciplinary team to ensure seamless care coordination and effective communication.
- Program Development and Implementation: Assist in the development and implementation of programs and policies to support the Mental and Behavioral Health Team.
- Master's Degree: A Master's degree in Social Work, Counseling, or Psychology is required.
- Experience: A minimum of 1 year of experience working with geriatric populations or in long-term care settings is preferred.
- Clinical Skills: Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills are essential.
- Communication Skills: Excellent verbal and written communication skills are required, as well as good interpersonal skills and the ability to work cooperatively on an interdisciplinary team.
