Job Summary

We are seeking a skilled RN Case Manager to join our team at Sutter Health. As a key member of our care coordination team, you will play a critical role in ensuring the timely and effective transition of patients through the healthcare system.

Responsibilities
  • Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' medical, emotional, and social needs to identify opportunities for care coordination and optimization.
  • Develop and maintain relationships with patients, families, and caregivers to ensure seamless transitions and optimal outcomes.
  • Utilize electronic medical records and other tools to track patient progress and identify areas for improvement.
  • Participate in quality improvement initiatives to enhance the overall quality of care and patient satisfaction.
Requirements
  • Current RN licensure in California.
  • Minimum of three years of experience in acute medical/surgical/ED/critical care nursing.
  • Master's degree in nursing, case management, or related field (or equivalent experience).
  • CCM certification (or willingness to obtain).
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
