ColumbiaCare Services is seeking a highly skilled and compassionate Intensive Case Management (ICM) Therapist to join our team in Medford, Oregon. As an ICM Therapist, you will play a vital role in providing diverse clinical approaches through a strength-based, person-centered, and trauma-informed lens to support adults experiencing severe and persistent mental illness.
Job SummaryThe ICM Therapist will work collaboratively with a multi-disciplinary field-based team to identify client needs, determine appropriate levels of support, and connect to community resources to resolve identified needs. You will perform comprehensive psychosocial mental health assessments, direct client-driven treatment, identify appropriate DSM diagnosis, and develop treatment plans with the goal of hospital diversion, increasing client self-sufficiency, and/or finding the most appropriate and least restrictive placement.
Responsibilities- Conduct comprehensive psychosocial mental health assessments
- Develop and implement treatment plans
- Provide therapy, case management, and skills training in various community settings
- Work collaboratively with a multi-disciplinary field-based team
- Identify client needs and determine appropriate levels of support
- Connect clients to community resources
- Master's degree in Psychology, Counseling, Social Work, or related field
- Eligible for Qualified Mental Health Professional (QMHP) registration
- Unlicensed therapists must be actively seeking or willing to obtain licensure
- Experience performing mental health assessments, diagnosis, treatment plans, and working with SPMI populations is preferred
