ICM Therapist

ColumbiaCare Services is seeking a highly skilled and compassionate ICM Therapist to join our team in Medford, Oregon. As an ICM Therapist, you will provide comprehensive psychosocial mental health assessments, develop treatment plans, and work collaboratively with a multi-disciplinary field-based team to identify client needs and connect to community resources.

• Conduct comprehensive psychosocial mental health assessments
• Develop treatment plans with the goal of hospital diversion, increasing client self-sufficiency, and/or finding most appropriate and least restrictive placement
• Work collaboratively with a multi-disciplinary field-based team to identify client needs and connect to community resources
• Provide therapy, case management, and skills training in various community settings to support adults who experience severe and persistent mental illness

• Master's Degree in Psychology, Counseling, Social Work, or related field
• Eligibility for Qualified Mental Health Professional (QMHP) registration
• Experience performing mental health assessments, diagnosis, treatment plans, and working with SPMI population is preferred
• Ability to pass a DHS criminal background check and a pre-employment drug screen
