Job Title: Resident Care Coordinator

We are seeking a compassionate and experienced Resident Care Coordinator to join our team at Cogir at the Quarry. As a key member of our healthcare team, you will be responsible for coordinating and delivering high-quality resident care services.

Job Summary:

The Resident Care Coordinator will work closely with residents, families, and staff to ensure that our residents receive the best possible care. This includes coordinating care planning, supervising care staff, and ensuring that resident records and charting are up to date.

Responsibilities:
  • Supervise and develop care staff to ensure high-quality care delivery
  • Coordinate admissions and move-ins, ensuring a smooth transition for residents
  • Ensure resident records and charting are current and accurate
  • Coordinate care planning with home health agencies and other external partners
  • Act as a liaison between residents, families, and staff to address concerns and requests
  • Assist with staff training and in-service schedules
  • Supervise the medication room and orders, working with Medication Aides and Caregivers
Requirements:
  • LPN/LVN nursing license in good standing
  • Minimum 3-5 years of geriatric nursing experience, including supervisory experience
  • Excellent verbal and written communication skills
  • Ability to manage time effectively and prioritize tasks
  • Current First Aid and CPR certification
