We are currently in search of several Medication Historian Pharmacy Technicians to become integral members of our Transitions of Care Pharmacy team. This team is highly esteemed and plays a vital role in enhancing the health of our patients.

Responsibilities

  • Execute independent tasks with precision and a keen eye for detail.
  • Identify patients being admitted through various entry points, including the emergency department.
  • Collect comprehensive medication histories from patients or their families in a professional manner, utilizing excellent interviewing techniques.
  • Compile a thorough list of patients' home medications, including over-the-counter drugs and herbal supplements, detailing drug names, dosages, frequencies, reasons for use, and last doses taken.
  • Provide exceptional service by addressing physician and staff inquiries, as well as patient and family requests.
  • Enter all medication history data into the electronic health record system.
  • Maintain records of all processed work for tracking and reporting purposes.
  • Actively engage in staff meetings to share information and address staff needs, concerns, and suggestions.
  • Practice within the full scope of your professional license.
  • Enhance your skill set and expand your pharmacy capabilities.
  • Integrate clinical pharmacy practice with advanced patient care processes.
  • Contribute to the growing trend of pharmacy technician specialists, paving the way for improved patient health outcomes.

Qualifications

  • High school diploma or equivalent (GED).
  • Valid Idaho Pharmacy Technician License.
  • A minimum of two (2) years of experience.
  • Certified Pharmacy Technician (CPhT) credential.
