We are currently in search of several Medication Historian Pharmacy Technicians to enhance our Transitions of Care Pharmacy team. This team plays a vital role in ensuring the health and safety of our patients.
**SIGNING BONUS**
Key Responsibilities
- Execute independent tasks with precision and a keen eye for detail.
- Identify patients being admitted through various entry points, including the emergency department.
- Collect comprehensive medication histories directly from patients or their families, utilizing exceptional interviewing techniques.
- Compile an exhaustive list of patients' home medications, including over-the-counter products and herbal supplements, detailing drug names, dosages, frequencies, reasons for use, and last doses taken.
- Provide exemplary service by addressing inquiries from physicians, staff, and patients or their families.
- Document all medication history information in the electronic health record system.
- Maintain records of all processed work for tracking and reporting purposes.
- Actively engage in staff meetings to share information and address staff needs, concerns, and suggestions.
- Practice within the full scope of your professional license.
- Enhance your skill set and expand your pharmacy practice capabilities.
- Integrate clinical pharmacy practices with advanced patient care methodologies.
- Contribute to the evolving field of pharmacy technician specialists, paving the way for improved patient health outcomes.
Qualifications
- High school diploma or equivalent (GED).
- Valid Idaho Pharmacy Technician License.
- A minimum of two (2) years of relevant experience.
- Certified Pharmacy Technician (CPhT) credential.
