Job Summary

The Associate Director of Social Services is a key member of our team at Vivent Health, responsible for providing support to the Director of Social Services and the department in areas related to case management, housing, and supportive services throughout the State of Wisconsin. This role involves staff recruitment, training, and supervision, program administration, grant compliance and reporting, quality improvement, and community relations and collaboration.

Key Responsibilities
  • Oversight of vital housing and supportive programs within the Social Services department, including supporting the team with securing affordable housing for patients/clients, developing permanent housing plans, and accessing insurance, government programs, and financial assistance to contribute to long-term stability.
  • Supervision and guidance to integrated case management, housing, and other social services staff on service delivery and programming, grant compliance, policies, and quality assurance activities.
  • Collaboration with State and City partners, HUD, and other funders, as well as community partners and internal departments, to ensure successful achievement of program outcomes and expectations.
  • Development and implementation of policies and procedures to ensure the effective operation of case management, housing, health insurance, and supportive programs and in compliance with all federal, state, local and agency service delivery standards.
  • Management oversight of supportive programs, including the recruitment and supervision of temporary staff or interns that provide these services.
  • Implementation of quality improvement initiatives identified by Vivent Health's Director of Quality and Senior Director of Patient Experience to achieve identified goals and high levels of client satisfaction.
  • Coordination of client engagement and communication strategies for collaborations with Vivent Health's Client Advisory Forum, and through surveys and focus groups, and promptly and effectively handle client complaints in accordance with the agency's Grievance Policy, as directed by the Senior Director of Patient Experience.
  • Networking with community resources throughout the state to ensure a continuum of care for clients.
  • Assistance with the preparation and submission of department work plans, grant reports, management reports and department budgets as required.

Requirements
  • Bachelor's degree in public health, community health, social work or other related field and/or five years of related experience.
  • Minimum of 2-4 years of supervisory experience required.
  • Exceptional interpersonal and communication skills, capable of effectively collaborating with diverse healthcare professionals and stakeholders.
  • Strategic mindset with the ability to develop and implement innovative solutions to complex patient care challenges.
  • Ability to lead organizational change and guide teams through periods of transformation while maintaining a positive work culture.

Salary Starting at $75k (Depending on Experience)
