POSITION SUMMARY

Under the guidance of the Outreach Manager, the Outreach Case Manager will engage with youth experiencing homelessness to foster strong, healthy relationships aimed at promoting self-sufficiency. This role serves as a vital link between the youth, the community, and local businesses, raising awareness about the services provided by The PLACE to enhance the health, safety, and well-being of homeless and runaway youth. Responsibilities include shelter coverage, street outreach, case management for both intensive and basic needs, and the facilitation of various groups and activities. This position emphasizes trauma-informed interactions, ensuring the physical and emotional safety of both youth and staff.

KEY RESPONSIBILITIES

The duties of this role are performed independently, in collaboration with the supervisor, and/or in coordination with other team members. Additional responsibilities may be assigned as needed.

All Outreach Case Managers will be accountable for the following general responsibilities:

  • Represent The PLACE within the community to educate and advocate for homelessness issues.
  • Collaborate with shelters to identify individuals in need of services and housing assistance.
  • Participate in all required meetings and client reviews, prepared to discuss client concerns.
  • Assist in conducting annual Point In Time surveys.
  • Engage in professional development through required training sessions.
  • Accompany youth on group outings for recreational and educational purposes.
  • Train and educate volunteers on effective engagement with homeless youth.
  • Actively connect with youth on the streets, in shelters, and through The PLACE programs.
  • Meet defined performance outcomes at both individual and team levels.
  • Provide health screenings, including HIV and HEP C testing, to clients.
  • Manage supplies, including tracking inventory and ensuring proper usage.

Case Management and Referrals

  • Work collaboratively with community service providers and The PLACE programs to ensure seamless service delivery.
  • Assist youth in securing permanent housing and family reunification, providing referrals for additional support services.
  • Accept referrals from partner agencies and collaborate to meet shared goals.
  • Conduct individual assessments and develop tailored case plans to address youth needs.

Supervision and Support

  • Build trusting relationships with homeless and runaway youth to facilitate their transition to stable housing or reunification.
  • Provide supervision through a trauma-informed lens, ensuring a safe environment for youth.
  • Guide youth towards appropriate referrals and foster relationships that may lead to case management.
  • Implement programs that encourage the development of healthy relationships.
  • Encourage youth participation in maintaining a clean environment.
  • Provide crisis intervention and support as needed.
  • Collaborate with team members to ensure coverage of all shelter areas during operational hours.

Documentation and Reporting

  • Maintain accurate records of services provided, including assessments and case notes.
  • Collect and manage data consistently to support program evaluation.
  • Ensure effective communication with staff regarding client status.
  • Uphold client confidentiality at all times.

Intake and Assessment

  • Conduct thorough intakes to establish rapport and trust with youth.
  • Evaluate the needs and intentions of youth, recommending appropriate program involvement.

In addition to the responsibilities outlined above, the Outreach Case Manager will also:

  • Provide resources to street youth through outreach efforts using a trauma-informed approach.
  • Work towards achieving established goals in collaboration with the supervisor.
  • Develop and implement programs that deliver resources and foster community among street youth.
  • Seek to build trusting relationships with youth who are not currently engaged with The PLACE.
  • Represent The PLACE at community events focused on human trafficking awareness.

PHYSICAL DEMANDS

  • Frequent standing, walking, and use of hands for various tasks.
  • Manual dexterity and coordination are required.
  • Effective communication skills in English are necessary.
  • Ability to operate department equipment, including computers and telephones.
  • Capacity to meet youth off-site as needed.
  • Ability to remain mobile for extended periods.
  • Capability to lift up to 50 lbs.

WORK ENVIRONMENT

This position requires extended periods of mobility in outdoor environments, which may include exposure to inclement weather and challenging conditions. Employees must be prepared to engage with youth in various public settings, including areas that may pose safety concerns. The PLACE provides necessary safety training and equipment to ensure employee protection in these environments.

QUALIFICATIONS

  • A Master's degree in social work, psychology, or a related field with at least two years of experience working with youth; or a Bachelor's degree in human services or a related field with a minimum of three years of experience; or a high school diploma with five years of full-time experience with high-risk youth.
  • Successful completion of federal and state background checks.
  • Experience in trauma-informed care or a willingness to adopt this approach.
  • Ability to effectively engage with the homeless youth community, particularly those who identify as LGBTQ or face other challenges.
  • Strong oral and written communication skills.
  • Negotiation and mediation skills, especially in sensitive situations.
  • Ability to work collaboratively within a multidisciplinary team.
  • Proficiency in Microsoft Office Suite.
  • Must be insurable to drive on behalf of The PLACE.
  • Attendance and punctuality are essential for this role.
