POSITION SUMMARY
Under the guidance of the Outreach Manager, the Outreach Case Manager will engage with youth in street settings to cultivate healthy relationships aimed at facilitating their journey towards self-sufficiency. This role serves as a vital link to educate youth, the community, and local businesses about the services available at The PLACE, promoting the health, safety, and well-being of homeless and runaway youth while assisting them in achieving permanent housing solutions. Responsibilities include shelter coverage, street outreach, case management for both intensive and basic needs, and leading groups, classes, and activities. This position prioritizes trauma-informed interactions, ensuring the physical and emotional safety of both youth and staff.
KEY RESPONSIBILITIES
- Represent The PLACE within the community to raise awareness about homelessness and advocate for educational and funding opportunities.
- Collaborate with shelters to identify individuals in need of services and housing assistance.
- Participate in required staff meetings and client reviews, prepared to discuss client matters.
- Assist in the annual Point In Time survey.
- Engage in professional development training as needed.
- Accompany youth on outings to various community resources and recreational activities.
- Educate and mentor volunteers on effectively working with at-risk youth.
- Actively engage with youth in various settings, including streets and shelters.
- Accountable for achieving defined outcomes at both individual and team levels.
- Provide health screenings, including HIV and HEP C testing, to clients.
- Manage inventory and supplies, ensuring proper use and storage.
Case Management and Referrals
- Work with community service providers and The PLACE programs to ensure seamless coordination and communication of services.
- Assist youth in securing permanent housing and family reunification, providing referrals for additional supportive services.
- Accept referrals from other organizations and collaborate with their staff to support shared goals.
- Conduct individual assessments and develop tailored case plans to address youth needs.
Milieu Supervision
- Build rapport with homeless and runaway youth to facilitate their transition from street life to stable housing or reunification with family.
- Provide supervision in a trauma-informed manner, ensuring a safe environment for youth.
- Guide youth in maintaining a clean and safe environment.
- Offer crisis intervention and support as needed.
- Collaborate with team members to ensure comprehensive coverage of the shelter.
Documentation
- Maintain accurate records of services provided, including assessments and case notes.
- Collect and manage data consistently.
- Ensure effective communication with colleagues regarding client information.
- Uphold client confidentiality at all times.
Intakes and Assessment
- Conduct thorough intakes to foster trust and comfort with youth.
- Evaluate youth needs and recommend appropriate program involvement.
PROFESSIONAL STANDARDS
- Adhere to all agency policies and maintain professional conduct at all times.
- Engage in continuous professional development and training.
- Promote a culturally sensitive and trauma-informed environment through respectful communication.
- Support youth involvement in decision-making processes.
WORK ENVIRONMENT
This position requires working in various outdoor settings, often in challenging conditions. Employees must be prepared to engage with youth in public spaces and navigate potential safety concerns. The PLACE provides necessary training and equipment to ensure employee safety during outreach efforts.

QUALIFICATIONS
- A Master's degree in social work, psychology, or a related field with at least two years of experience working with youth; or a Bachelor's degree in human services or a related field with a minimum of three years of experience; or a high school diploma with five years of experience working with high-risk youth.
- Successful completion of federal and state background checks.
- Experience in trauma-informed care is essential.
- Ability to effectively engage with diverse youth populations, including those who are LGBTQ or facing mental health and substance use challenges.
- Strong communication skills, both verbal and written.
- Ability to work collaboratively in a multi-disciplinary team environment.
- Proficiency in Microsoft Office Suite and basic computer skills.
