POSITION SUMMARY

Under the guidance of the Outreach Manager, the Outreach Case Manager is tasked with engaging with youth in street environments to foster healthy, supportive relationships aimed at promoting self-sufficiency. This role serves as a vital link to inform youth, the community, and local businesses about the services provided by The PLACE, thereby enhancing the health, safety, and well-being of homeless and runaway youth and assisting them in exiting street life permanently. Responsibilities include shelter coverage, street outreach, case management for both intensive and basic needs, and facilitating groups, classes, and activities. This position prioritizes trauma-informed interactions, ensuring the physical and emotional safety of both youth and staff.

KEY RESPONSIBILITIES

  • Represent The PLACE within the community to raise awareness about homelessness, fostering education, funding, and advocacy.
  • Collaborate with shelters to identify homeless individuals requiring assistance in accessing services and housing.
  • Participate in mandatory staff meetings and client reviews, prepared to discuss client-related issues.
  • Assist in conducting annual Point In Time surveys.
  • Engage in professional development training as required.
  • Accompany youth on group outings to various recreational activities.
  • Educate and mentor volunteers on effective engagement with homeless youth.
  • Actively connect with youth on the streets, in shelters, and through other programs offered by The PLACE.
  • Ensure accountability for defined outcomes at both individual and team levels.
  • Provide HIV and HEP C testing for clients of The PLACE.
  • Manage supplies, including ordering and ensuring proper use and storage.

Case Management and Referrals

  • Work in partnership with community homeless service providers and The PLACE programs to ensure seamless coordination and continuity of services.
  • Assist youth in securing permanent housing or family reunification, and provide referrals for supportive services to maintain stability.
  • Accept referrals from other agencies and collaborate with their staff to support shared goals.
  • Conduct individual assessments and develop tailored case plans to address the needs of participating youth.

Milieu Supervision

  • Build relationships with homeless and runaway youth to encourage their transition from street life to stable housing or reunification with family or support networks.
  • Provide milieu supervision using a trauma-informed approach, ensuring a safe environment for youth.
  • Guide youth in accessing referrals and foster relationships that may evolve into case management.
  • Facilitate programming that promotes the development of healthy relationships.
  • Encourage youth to maintain cleanliness in their surroundings.
  • Offer de-escalation and crisis intervention as needed.
  • Collaborate with the team to ensure comprehensive coverage of all shelter areas during operational hours.

Documentation

  • Maintain accurate records of all services provided, including assessments, case notes, and service plans.
  • Collect and manage data consistently.
  • Communicate effectively with staff regarding client matters.
  • Uphold client confidentiality at all times.

Intakes and Assessment

  • Conduct thorough intakes to establish trust and comfort with youth.
  • Evaluate the needs and intentions of youth, making recommendations for their involvement in additional programs.

WORK ENVIRONMENT

This position requires long hours in outdoor environments, often facing challenging physical conditions. Employees may encounter situations involving interpersonal contact that can be aggressive or threatening. Comfort in providing essential supplies to youth in need is crucial. The PLACE provides necessary personal protective equipment and safety training to ensure employee safety in these unpredictable environments.

QUALIFICATIONS

  • A Master's degree in social work, psychology, or a related field with at least two years of experience working with youth; or a Bachelor's degree in human services or a related field with a minimum of three years of experience; or a high school diploma with five years of full-time experience working with high-risk youth.
  • Successful completion of federal and state background checks.
  • Experience in trauma-informed care or a willingness to learn.
  • Ability to effectively engage with the homeless youth community, including those who identify as LGBTQ or face other challenges.
  • Strong oral and written communication skills.
  • Negotiation and mediation skills, particularly in sensitive situations.
  • Ability to maintain confidentiality while making sound decisions.
  • Experience working within a multi-disciplinary team and with various agencies.
  • Proficiency in Microsoft Office Suite.
  • Must be insurable to drive agency vehicles.
  • Attendance and punctuality are essential for this role.
